Collection of Personal Data

By submitting this form, you represent and warrant that all the information you have provided is accurate at the time of submission. In the event of any changes to your information, you undertake to inform the Public Hygiene Council (PHC) of any such changes in writing immediately. You agree to be bound by PHC’s Privacy Policy, as amended from time to time, which outlines how PHC manages your personal data in accordance with the Personal Data Protection Act 2012, and you will be deemed to have consented to PHC’s collection, insofar as is permissible by the applicable law, PHC shall not be responsible for and shall not be liable for any damage or loss any participant may incur and/or suffer, whether or not such damage or loss is caused by the organiser’s negligence or otherwise.

Acknowledgement of Risk and Undertaking

• I confirm that I understand the nature of the event and/or activities and its inherent risks.
• I agree to cooperate fully with the organiser/PHC and I undertake to diligently comply with all safety instructions.
• I also undertake not to hold PHC, its staff, members, any other relevant personnel or authorised agents responsible for
  1. any damage to or loss of property; or
  2. any injury or loss of life, which may be sustained by me during the event, including where transportation is provided whilst in transit or arising from the event and in the course of transportation provided.
• Provided that such damage to or loss of property, or any injury or loss of life is not caused by the negligence of PHC, its staff, members, any other relevant personnel or authorised agents.
• I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby.

Photography and Videography Consent

• I understand that Public Hygiene Council (PHC) its officers, agents, employees, assigns, licensees, successors and its photographer(s) and/or videographer(s) (collectively known as the “PHC Producers”) may take photographs, videos or sound recordings of me during my participation in the event.
• I hereby assign to PHC the ownership and rights to use, distribute, exhibit, copy, or publish my image, likeness and sound of my voice as recorded on photographic, videographic, or electronic devices, without monetary compensation or compensation in kind.
• I waive the right to inspect or approve the finished product wherein my image or likeness, or sound of my voice appears.
• Additionally, I waive all claims to copyright or other intellectual property ownership including any right to royalties or other compensation arising or related to the use of my image, likeness, or voice recording.
• I understand that photographic, videographic, or audio recordings may be used for the following purposes:
  • conference presentations
  • educational presentations or videos
  • informational presentations
  • social media content
  • printed collaterals
  • internal corporate use in documentaries, training videos, or internal staff communications
• By acknowledging this clause, I understand that:
  • This permission signifies that photographic, videographic, and audio recordings of me may be reproduced in any manner, for an unlimited number of times in perpetuity, and in any online or offline medium or media. There is no time limit on the validity of this release nor any geographic or territorial limitation on where these materials may be distributed.
